Counselling Appointment Request

As a first time client, I understand and agree a $50 deposit by credit card is required at the time of an appointment confirmation.  Further, I understand the entire deposit will be applied to my initial consultation fee.
I am requesting a personal appointment with Diane McLaren but I am not assured of my preferred day/time indicated above until I receive direct confirmation from Healthy You Naturally who may offer alternate days/times.
I acknowledge that once I have a confirmed appointment, if I fail to arrive at the agreed appointment I forfeit my deposit as payment for the scheduled time in leu.
I acknowledge and agree that if I fail to give 2 business days notice of my need to reschedule a confirmed appointment, I may be assessed a penalty fee.
I understand a satisfaction guarantee applies only to a first time client and should I be dissatisfied with the counselling service within the initial 20 minutes, I can cancel immediately and demand a full refund without reason.
I understand that by completing this request, I am not obligated in anyway until my appointment is confirmed and the deposit is paid.

General Conditions Of A Personal Appointment

When you book your appointment, that time is held exclusively for you.  If you want to reschedule please provide at least 2 business days notice to avoid penalty.  This allows us time to make your time available to another client.  Thank for your understanding.

Appointments apply to face-to-face and by-phone arrangements.  We maintain strict confidentiality on all information and our policy is to never give, share or sell personal information.
