Online Appointment Request

Please complete each field of this form to request an appointment.  We will contact you to confirm or adjust the day/time:

1) Online requests will be verbally validated prior to confirmed booking.
2) First time clients at time of our call to confirmation, must deposit $50 to validate their appointment, only processed at time of appointment.
3) First appointment client guarantee applies: cancel within 20 minutes of arrival at appointment, and the deposit is waived/no charges apply.
4) In the event of a no-show for a scheduled appointment, the deposit will be processed as payment for time scheduled in leu.
5) Where a confirmed appointment must be rescheduled, a 2 business day notice is required to avoid possible penalty charge.

When you request your appointment, upon confirmation, that time is held exclusively for you .  If you need to reschedule, please provide at least 2 business days notice by calling (905) 271-1180 to avoid possible penalty.  This allows us time to make your time available to someone else who wants it.  We thank for your understanding.  Appointments apply to in-office and by-phone arrangements.
